Each Tastylia 20mg unit contains 20 mg of tadalafil: the label's maximum recommended as-needed dose. The label's usual starting dose is 10 mg (FDA label §2.1).
How tadalafil works
Tadalafil blocks the enzyme phosphodiesterase type 5 (PDE5). During sexual stimulation, nitric oxide released in the penis raises levels of cGMP, which relaxes smooth muscle and lets more blood flow into the erectile tissue. Blocking PDE5 increases the amount of cGMP, which enhances erectile function. Because sexual stimulation is needed to release nitric oxide in the first place, tadalafil has no effect without it. In a stopwatch study of 223 men, 52% of those taking tadalafil 20 mg reported a successful erection at or before 30 minutes after dosing, compared with 38% on 10 mg and 35% on placebo. (FDA label §14.1)

Who should not take it
- Using any form of organic nitrate, regularly or occasionally. Tadalafil increases the blood-pressure-lowering effect of nitrates. If a nitrate is needed in a life-threatening situation, at least 48 hours should have passed since the last tadalafil dose. (FDA label §4.1)
- A known serious hypersensitivity reaction to tadalafil (CIALIS or ADCIRCA). (FDA label §4.2)
- Taking a guanylate cyclase (GC) stimulator such as riociguat. (FDA label §4.3)
Warnings
- Heart health. Not for men whose heart condition makes sexual activity inadvisable. (FDA label §5.1)
- Erection lasting more than 4 hours. Seek emergency medical attention, whether the erection is painful or not. (FDA label §5.3)
- Sudden vision loss. Stop and seek medical care for a sudden loss of vision in one or both eyes, which can be a sign of NAION. (FDA label §5.4)
- Sudden hearing loss. Stop and seek prompt medical attention for a sudden decrease or loss of hearing. (FDA label §5.5)
- Low blood pressure. Taking tadalafil with alpha-blockers, blood-pressure medicines or substantial alcohol (5 units or more) may lead to low blood pressure. (FDA label §5.6, 5.9)

How quickly does Tastylia 20mg work?+
Tadalafil reaches its peak blood level between 30 minutes and 6 hours after a dose, with a median of 2 hours (FDA label §12.3). In one study, 52% of men taking tadalafil 20 mg reported a successful erection within 30 minutes, compared with 35% on placebo (§14.1).
Can Tastylia 20mg be taken with food?+
Yes. The label says tadalafil may be taken without regard to food (FDA label §2.5).
How often can Tastylia 20mg be taken?+
For as-needed use, not more than once per day in most patients (FDA label §2.1). Lower limits apply to people with kidney or liver impairment, and to those taking potent CYP3A4 inhibitors such as ketoconazole or ritonavir: no more than 10 mg once every 72 hours with those inhibitors (§2.6, §2.7).
Can Tastylia 20mg be taken with nitrates?+
No. Tadalafil is contraindicated with any form of organic nitrate because it increases their blood-pressure-lowering effect (FDA label §4.1). If a nitrate is needed in a life-threatening situation, at least 48 hours should have passed since the last tadalafil dose.
Can you drink alcohol with Tastylia 20mg?+
The label warns that substantial alcohol (5 units or more) combined with tadalafil can increase the potential for orthostatic signs and symptoms, and that it may lead to low blood pressure (FDA label §5.9).
What are the most common side effects of Tastylia 20mg?+
In the as-needed studies, the most common side effects at 20 mg were headache (15%), indigestion (10%), back pain (6%), and muscle aches, nasal congestion, flushing and pain in limb (3% each) (FDA label §6.1, Table 1).
